Default Fluctuating idle and hesitation around 2500-3500 - Causes?

I got another tech question on Miataforumz.com that I can't answer and I am looking for guidance. Here it is:

Any ideas, my idle goes up and down (300 to 900) and have a marked hesitation around 2500 to 3500 along with high petrol consumption. ICV replaced with known good: plugs & leads new; TPS new; TB cleaned; EGR ok and cleaned. No codes!! Have ordered O2 sensor + temp sender (back of engine). This is a 2004 NBFL European spec (Etna model)

I told him to check for vacuum leaks and he indicates he can't find any. What else should we tell him? Please help me help him.
