Default Innovate Wideband issue (E9)

Hello, first of all I'm not a 100% this is the right category to ask this question, so if not let me know and I'll move it

I have a 1.6 NA MX5 with diy turbo kit. Drove great for a year however now I'm having issues with my Innovate mtx-l plus wideband. I'll try to describe the behavior as best as I can.

When starting it would show code E9 (voltage issue) and while driving it would sometimes cause strange behavior like starting the heating cycle again, turning off, showing E9 and then all of a sudden it was fine again.
Checked al the wires and voltages (notes: 12v to cigarette lighter, ground to engine ground) they all checked out fine.
Bought a new sensor calibrated it and set it up (someone advised me that faulty sensor might cause strange behavior from the wideband gauge), but the issue stayed.
Tested all the wiring and redid all the wiring yesterday. What I noticed was when using 12v from the cigarette lighter it would show the E9 code and when using 12v from the radio it did not. So I redid the 12v to the radio 12v.

Went for a test drive worked great again, tested it a few more times because I couldn't believe it; still fine no codes. Went to bed all content with myself for fixing the issue.

Woke up today went to the grocery store and when trying to start it after doing groceries it showed E9 again. Started it and it went from heater to e9 to heater to e9 etc. Engine was running rough. Eventually it started working normal again. Drove home, shut it of, started it again .... no error codes
Let it cool down for two hours, started it again... no error codes

I have absolutely no idea what could be causing this except for a faulty wideband unit. But too me it seems strange that a wideband unit dies after one year with like 3000km
So I thought lets ask the forums before I buy another expensive gauge (probably an AEM this time)

So if anyone has any similar stories, ideas what else to check or ideas on what it could be that would be great.

I had nothing but issues with MTX-Ls till I wired it as per the instructions (funny that hey?).
Wire power and ground direct off the battery to a relay. These things are so sensitive to voltage and RF. Wire it properly.

Wow I just looked up the manual and I completly missed that page. Seeing as they are so picky with voltage that might be it.
I'll rewire that this week and see if that fixes the issue.

Don't be surprised if the E9 errors have fried its mushy brain already. But see how you go.
